Who are the service providers?
The plan has been tailored and calculated based solely on services provided by Hospital Chiriqui, S. A. However, any service provided by any other medical facility, the affiliate and his dependants must paid in full all expenses incurred thereby.

then be reimbursed, as if expenses were generated at Hospital Chiriqui. No coverage is granted overseas.
What are the enrollment requirements?
The applicant must respond to all questions in the application
and in the proof of insurability. Include inscription fee and
the first month.
Which procedures require Pre-Authorization?
All hospitalization admissions and non-routinely special medical
procedures routinely of high cost and involving great technology, whether they call for hospitalization or emergency services, Pre-Authorization are required. Likewise, all elective surgical procedures (non-emergencies) require a second surgeon opinion.
All E. R. admissions must be reported within 24 hours following
the admission, even when the patient has been released. In the event the admission is pre-authorized and the patient remains hospitalized beyond the P-A time, MS Chiriqui will not honor to defray for those extra days. The patient is responsible for any extra-day expense. Any surgical procedure non-related to the original diagnosis incurred by the patient at Hospital Chiriqui, for which Pre-Authorization was granted, is not be covered by MS Chiriqui.
What are the limitations?
This plan does not cover any pre-existing condition before 2 full years of enrollment. A pre-existing condition is defined as any illness, injury or condition for which a person is diagnosed,
received medical treatment, services or supplements. Also, it
could be the presence of synthoms that cause a wise person to seek a doctor opinion, medical care or treatment. |
